What the Incident Investigation module covers

On the Alberta NCSO exam, Incident Investigation covers root cause analysis, evidence preservation, witness interviews, reporting obligations and corrective actions. It is one of ten modules, and the exam draws across all of them — which is why candidates who are strong on site but narrow in coverage still fail.

An 'Accident' is defined as:

Why

An accident is UNPLANNED and UNDESIRED - causing: property/equipment damage, personal injury, death, or occupational illness. 'Unplanned' distinguishes accidents from intentional harm.

Why

Investigation exists for ONE reason: PREVENTION. Finding root causes allows the organization to fix systemic problems. Blame-focused investigations miss root causes - incidents recur. The exam will test this philosophy repeatedly.

Who should be involved in an incident investigation?

  1. Only the safety officer and human resources
  2. The employer (or supervisor) AND a worker representative - plus subject matter experts as needed correct
  3. Only managers and senior supervisors
  4. Only government OHS officers after formal notification
Why

Investigations should include management/supervision AND a worker representative. Workers have firsthand knowledge of actual conditions. Worker involvement improves the quality of findings and builds trust in the process.
